When should I file a Worker’s Comp claim?

If you ever injured onsite, even trivial injuries should be reported.

Why should I file a Worker’s Comp claim?

We want to make sure all of our employees are safe and in the event you are injured while working it is important to report your injuries, so we can ensure you are getting the care and support you need for your injury.

What are the steps to file a Worker’s Comp claim?

Within 2 hours of the injury, let HR know you have been injured and complete an Accident Investigation Form. 

Ensure your manager is aware of the injury right away as well.  They must complete a form regarding your injury.

Log into ADP>Resources>Forms Library>Accident Investigation Form

I am a manager; do I have to file a report?

Yes, you do need to file a report on the behalf of your employee

Where do I find the Worker’s Comp Form for manager?

Log into ADP>Resources>Manager Tools>Accident Investigation Form

Once you click on the link, enter your name and [email protected] email where prompted.  This ensures the report gets to HR in a timely manner

If got to see my doctor what do I need to report back to HR?

You will need to provide a note from your doctor to release you to full work duty or any work restrictions they place on your injury.

I have a follow-up doctors visit, what do I do?

After each visit you will have to inform HR of any updates or changes to your work restrictions.  You will need a doctor’s note for this as well.

What happens after I file a claim?

HR will have weekly meetings with you and send follow-up communications to your manager until you are released for regular work duties.
